Medical Information

This is the most important part of the Ice Cube System.  The more information that you provide, the better your chances of survival if you become incapacitated.

Enter your name and the city where you reside.  To keep your information secure, there is no requirement for street address.

Enter your primary care doctor's name and phone number.  In the "Ice Contact" box, enter the individual that should be notified if you are sick or injured.

Your photo will properly identify you to paramedics and Emergency Room personnel.  To add your photo, merely click on "Change Photo" and locate the digital file on your computer.  It will automatically be copied to the Ice Cube.

Enter your medication (with dosages), allergies to medications or other substances, implants or other "hidden" devices in your body, etc.  There is no limit the the amount of information that you can enter in each box.

After completion of the above, print out an ID Card.  Use a heavier stock paper and a color printer if available.  This ID Card should be carried in your wallet or purse.

 

 

South By Southwest will not see or otherwise access your personal information.  Your personal information will not be transmitted by this software.

The Ice Cube is not a covered entity according to the HIPAA guidelines.  Legal opinion here.
